After following the step by step instructions for creating an advanced form, I am still receiving script error it's as if the submit button does not know where to locate the thanks you page I created.

I have a referral form saved as .html, & .php
I have saved thank you page as .html,& .php
I also have a mailto on submit, that isn't functioning as expected.
