you can use a 3rd party "contact us" form. if you want one for free and don't mind them adverting on your page, then do it this way. OR pay for it and you can customize it to your liking. (which is easiest way, google "free contact us forms" and look around...hint hint.)

OR you can do this via your cPanel to add a data base to your server to start setting up your contact us forms. follow the directions the 3rd party gives you.
