Well, without much scripting, I've seen a few form makers through CPanel's Fantastico, if it helps.

I use a poll and other tools from www.webpasties.com - you can try free for a month, and then for a small sum (£19.99 to me - not sure what that is in dollars) can use all the tools for 6 months. I have had a number of polls now - extremely easy to use, and I change them often. The benefit of this tool is that it doesn't have adverts all over it, or cause pop ups!
