404 error html on screen

I recently changed the background color and heading on all my pages. With doing this all the pages are working correctly except www.ocirwest.net/quoterequest. This is a vital page to my business and can not be working incorrectly, as you will see when you open the page there is html code at the top of the page. I need assistance with getting this rectified asap.

Re-publish your "Quote Request" page as "--.php" according to the detailed instructions outlined in the Tutorial.

Use BueFTP to delete the "--.html" page version from the server (from your Root, or "public_html/" Directory), and be sure to also re-publish all other web pages with the corrected navigation to the proper page with the proper extension.
