Much better! Ties in really nice. One suggestion- the Home button looks out of place? Maybe try something else? Maybe use the shape tool and create an elipse and try different tones/colors and then text Home on top and make a hyper-link? Just a thought.

Contact page looks good. "Survey" might throw a few people off though.

But the form it self looks good.

Your site looks good but I think people might not want right away to fill out a survey and to find the home button was a little different to see it just stuck there. Can you change the color?
