Thread: Form Help Please

And my forms "Last_name" box and the "First_Name" box do not work , when I send email to my self to see if it works it reads exactly ( with empty spots )

Values submitted from web site form :
comments : Test message
Submit : Submit
Client : Yes
Also what is the way to modify check box so it looks like the rest of the
form

These three fieds, email, first name and last name are slightly out of the form on the right side, so they are NOT part of the form and for this reason ignored. Selest them, and move them slightly to the left. The comments textarea is just inside.

I am not sure what you mean about the checkbox. Checkboxes as well as raio buttons are as you see them by definition, you cannot change them.
