Thread: Radio Buttons

I am trying to create a survey on my site using a form ! is this correct?
when I view the page all the radio buttons can be selected ie when I require a multiple choice one answer yes or no you are able to select both

Set the "name" attribute on both radios buttons to be the same for each group of yes and no's.
