Default html code showing on top of page

Hiya, having some problems with form. I have added a form for testing but keep getting an html code at the top of the page. Also do not recieve any emails. I have used the built in php. When I used it in the old BV it was fine. Any ideas what I am doing wrong?

As it is stated in the Form Wizard Tutorial and the new Forms Tutorial, IF you use the embedded form processor, the form page needs to be published as php and not as html.

Of course, you will need to update your menus that point to this page.

I ran into this same issue, the problem for me was my submit button was not associated to my form. It looked like it was but if fact was a separate design element on the page, let us know!
