Okay... so, I have a MySQL database, login name, and password created. And I have Form Maker Pro all installed. Please tell me exactly what the installation form wants for MySQL host? And... is MySQL login the same as the login name I created? Thanks

MySQL host : localhost The MySQL login is : cpusername_loginname where cpusername is your Control Panel username and loginname is the user you created for this Database. Make sure you have granted this user ALL permissions.
