Email not providing all info captured in form

Hiya. Hopefully this is an easy one. I've seen other posts about this issue, but it's still not working for me.

I have a simple form on my website. I receive the email just fine and the basic information. But, for some reason it's not capturing the comments ("description of services needed") in the email.

Thoughts??
