Default File upload

I have a form on one of my sites and it has 7 file upload buttons, the form works perfectly apart from I only receive 4 of the files that have been uploaded.

Anyone know why?

How is the form made? where is it?

We can't be of much help if we don't know what we are talking about.
By guess, you have 3 upload fields named with the same names as other three.

5 out of your 7 upload fields are un-named. Name them (i.e. file1, file2, etc...) save - publish - refresh before testing.
