Default can not upload anything in php

I was just trying to add form to my site but i cant upload it.
It is not showing no matter how many times i am refreshing.
It says it was uploaded but i can not see it.
I was trying from the scratch and based on the template.
I even have done it on my other website but before last version of BV. www.bdconstruction.net
In order to upload it i have to change to html but if i do that i can see form but i does not work. And my page looks like that www.ny-creativedesign.com/contactus.html

if i try to submit that form i am getting:
Not Found

The requested URL /< was not found on this server.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.




It used to be so easy, what has changed as far as php uploading goes?

hm, thank you. Now why i could not get that to work.
One more quick question. If i want to add survey form that will appear on every link for this website do i have to publish them all as php? Is there anything different for index page?

ah nevermind, i got it, it help me realize priorities when it comes to links, and i have deleted html index files out of my ftp. Works perfect now. Thank you so much.
