Problem with form not sending

Hi Folks,

I have created an application form that sends perfectly but does not send the data on the form to the email.

When I test the form to my email I only receive a blank page.

The entry form that I use as an example is found at http://www.gnjgf.co.za/entryform.php and th form that I want to work similaryly is found out http://www.gnjgf.co.za/application.php

I would appreiciate it if someone could veiw the later page and let me know what is wrong?
