new form not seen on page.

i have build this page that has a simple form on it from the form wizard

as you can see the source code does not say anything about a form but the html generator of my built page does. here is the code generated:

now i have cleaned my caches, and also tried to see it on another computor but the form is not there. what do i have to do?

Not sure what you mean. The code DOES contain a form. Please clarify.

this is where the form should show up but it doesnt. the code i took is from the website builder generated code. Why doesnt the form show up with the published page?

I can't answer this one. It depends on something in your page, simply seeing that the form is not there doesn't answer on why.
