Default Form published as HTML not PHP?

My feedback form published as HTML not PHP.

When I pull it up on a browser, and tag it as HTML, it finds it, but has a bunch of code on top of the screen. When I try to pull it up as PHP, it does not find it.

http://www.surveys.wx4usa.net/survey.html is my page in question.

Thanks everyone for all of your help!!

Your page IS published as html, whilst it SHOULD have been published as php.

The reason the code appears is because it is html

View > Page properties and set the page extension to be php.
Save, publish, change the link to your menus to point to the php page.
You will need to delete the html page from the server, and republish the pages where you change the menus to point to the php page.

The default extension is set to publish as .html and only needs to be changed if your page contains PHP.

You will need to reset it in page properties as oulined above.
