Make sure you have typed in your success page url correclty. Remember to add the .html after your success page title. The same with your error page.

well, what is your success page name?
How did you put it in the form properties?

Did you include your website name....eg....http://www.dvd-demos.com/success_page.html

what is meant to be placed in the "action" box in page properties because the exact information that is in there now appears after my website name when the "website page not found" page appears

I don't know that part of the page properties so well. Maybe you should delete it..... as far as I know, you are struggeling with the form. I don't think page properties has anything to do with forms...(I am speaking under correction)

Make sure you have specified the form action in the form properties window.

If nothing works, then I will hold thumbs for someone to come along who will be able to solve your problem...

Your contact page is published as html. It needs to be php. Go to Page Properties, and set the page extension to be php. Save, Republish, refresh your browser before testing.
