    Default Help with Form Wizard

    Hi,

    I've created a new page called survey.php and all input works fine. On the feedback report I get in an email it does not have the same label identities as the original survey, for example on our survey... your name shows as editbox1 on the feedback report, another question on our survey comes back as ''On'' it doesn't say yes or no.

    Can you advise me where and how to edit these fields? Thanks for your time.

    You must NAME your fields. Double click each field and give them the correct names.

    For Checkboxes the default value is ON, change it to whatever you need it to be.
