Default Captcha BV11

Is anyone else having problems with CAPTCHA is BV11?

When i add it to my form and publish it i get an image cannot be found where the CAPTCHA should be

Anyone any ideas?

I have the same problem. When I take the page live, the image is not displaying. I might have to resort to something like a math question until an answer is available
