I cannot access my form pages online

I have published my website www.enitiate.co.za. I can access all other pages, but I get error 404 when I try and access two form pages www.enitiate.co.za/contact_form/html and www.enitiate.co.za/visibility_test/html. I am not sure where I went wrong. Please help.

I have a fast connection but Im having difficulty bringing your site up.

To answer your question though when you create a page with a form on it the page extension is changed to .php instead of .html

I waited 5 mins for your site to load and it didnt so I gave up...sorry

Oh, no! That is not good. My header banner's size was too big but I got it down under 50KB. I know that it is suggested a web page must not exceed 100KB, but I do not how to check it. Can you help me?
