Thread: ComboBox Question

I would like to create something very similiar to the "Find a Certified Music Teacher" section (see above link). The person clicks their location (or zip code), and clicks go, and the page pulls up all the specifics based on what they entered.

Can someone please point me in the right direction to begin creating this type of form?

I am going to take the silence as there is no way to do what I described, or I make no sense.
