Finally I succeeded to upload the fmp folder (including all subfolders), because not all subfolders do automatically upload!).

so I am currently at the installation process:
http://www.artrental.biz/fmp/install.php

Now rises the next problem. With all the passwords we need to have at vodahost, I am completely lost:
Requested is:
Username: ("is this my loginname for the cPanel login?; or to login the community?) Should it be "Berry", or "jhjysegj"
Email: I think I can use any valid email adress, so that is no problem.
Password: related to the username, so no problem for me

My SQL host: this should be my MySQL database full path; what is meant by this? which full path? (from the browser?)
My SQL login: is it my username including name>? "jhjysegj_Berry" or only "Berry"?
My SQL Password: related to the above.
My SQL database: should be: "jhjysegj_Artrental"??


sorry, but I am a bit lost with all the usernames and logins. I saved them all, however to know which is to be used at which field is still a bit difficult....

Just "localhost", nothing else. No IP address is required.

Yes, you use the password set for the user. Only one word.

Yes, you need the complete DB name.

I see that you have set both the DB name as well as the user name to be the same, jhjysegj_artrental, correct?
Regarding your last question, you need both the username and database name, as a database may have more than one users, with different privileges.

This can be whatever you want, it is user defined. Just make sure to remenber it as you may be asked for it when entering your FMP admin area.

Just type in there " localhost "

How have you set it ? usually it is "cpusername_setusername"
If you have set the MySQL user to be "berry", and your cp username is jhjysegj, your Database username (loggin) is "jhjysegj_berry"

My SQL Password: related to the above.
Correct, just one word, the same that you have set as password for the Database user.

My SQL database: should be: "jhjysegj_Artrental"??
correct. Take care it is case sinsitive.

I managed to get FMP installed, but both the Sites tab and the Data Storage tab just link me to empty pages. As far as I can see, all my files are copied. What else can be wrong?

PS. Can anyone tell me please how this forum thing works. Should I post a new thread when I have a question and how do do it? I only figured out how to reply to posts.

i get the same error message :Template Error: set_root: tpl is not a directory.Halted. as most people. But on checking the FMP file and subfiles all seem to have been coppied. I have also set up a database.

I already deleted and reinstalled the FMP file
I changed the CHMOD to 755

Any ideas ? (its just that I havent got much hair left)

Typical error when not all of the files and folders have been uploaded.
If you have used BlueFTP to upload the files, it has the bad habbit to miss entire files when you drag and drop the whole folder. Check again and you will see that something is missing.

Yes, Form Maker pro is only available to VodaHost Clients. However,
Advanced BlueVoda Form Processor is available to everyone, and the files for non BV users are also available in the download zip.

I went to treasure chest and tried to dowload form maker pro but after downloading, it can't open the archive file..

It seems that the zip file is not valid.

No, the zip is ok. If you have tried to download FMP and the download failed for any reason, it leaves traces that can then prevent the correct download. Try clearing your internet temp files: IE, Tools, Options, Delete temp files. Then try downloading again.

The FMP folder has to have permissions set to 755. Please do so, using either FTP or File Manager, and it will work.

This is a typical error which occurs when not all files have been uploaded. If you have used BlueFTP, it (almost for sure) has left out some folders/files. Check it.

The server encountered an internal error or misconfiguration and was unable to complete your request. Please contact the server administrator, webmaster@naturaltherapynetwork.net and inform them of the time the error occurred, and anything you might have done that may have caused the error.
More information about this error may be available in the server error log.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

Check your FMP folder permissions. It must be set on 755. If you have it set in anything else, you will get this error. You also have mail

Hi I am also wondering where I find the mysql database pathway to fill in my install form. I have read the instructions and created the new database. I have looked in the cpanel and don't see where it is.

I did fix error pages thanks to these posts thank you. Not all my files had been transfered.

Let's suppose that you have called your database "fmp" when you created it. Let's also suppose (it is necessary to do this) that you have created and added a user in your database, and this user is called "kell" . Let's also suppose that when creating this user, you have set the password: "1234"

Your path is : localhost
your DB name is "username_fmp" where username is your CP username.
Your DB user is "username_kell"
your password is whatever you have set it, in this example "1234"

I've read every post above and I got through almost to success but then I get an error once the form has been completed and submitted.
I requested both a Preview and Thank you page. Once the form has been filled I get this - this error is with the Preview page removed as thought that may be a problem so this is just with the Thanks page:

Hey, I have everything installed correctly, and everythings going great...I made my form, and it's on my site and everything, but when I fill in all the fields (Name, E-Mail, Comment), and press Submit, I get this message;

"
Internal Server Error

The server encountered an internal error or misconfiguration and was unable to complete your request. Please contact the server administrator, webmaster@rancidpunx.net and inform them of the time the error occurred, and anything you might have done that may have caused the error.
More information about this error may be available in the server error log.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request."



Somewhere around Page 2 of this thread, someone else had the same problem, but I couldn't find a solid solution (except for re-installing formprocesorpro.php, which I did and nothing changed). I'd delete everything and retry it all, but I've already spent about 3 hours on it, so...if anyone can help, that'd be great.

As already writen above, it is for sure a permissions issue of a single file (depending on how you have setup your form, it could be the file formprocessorpro.php in the "forms" folder, inside the "fmp" folder), or even others, or the entire folder.

The formprocessorpro.php permissions shoul be set to 644

I have followed Aliens info on page 1 #30 but still keep getting IE cannot display web page.

Username:cpanel user name
email:my email
password:cpanel pass
confirm:cpanel pass

SQLhost:192.168.1.% (tried with and without the %)
SQL login:I tried just the user created as well as cpanel user_createdname
SQL password: tried cpanel password and created password
SQL DB:cpaneluser_created DB

Can anyone help?

SQL host = localhost

SQL loggin = cpusername_databaseusername
SQL Pass = the password you have provided when you create the user for the database

Of course, when you create a user for the database, you also need to ADD this user to the database you have created for FMP

Also, tomorrow the new ABVFP is being launched, with most of the features of FMP

the new ABVFP will have almost everything that FMP has. But, it alloes for the freedom in form design: you build the form in BV, so you have an absolute layout control

Does anyone know of a way within FM Pro to redirect back to the originating page rather than a fixed Redirect URL?
I have a "Tell a Friend" form which is part of the footer in each page and as part of the email that's sent to the Friend it sends the exact URL where the form was filled out so need to get back to the same page again.

Hi, I've had prob like error 404. Had just solved it. (Yeah!) I think it is really straight down to OLD FASHION way of checking and making sure ALL files were upload to right side of FTP manager.

My style: i took a long ruler and literally check ONE BY ONE just to make sure ALL files were copied to right side of FTP manager. Realised some files were not copied over (Dunno why like that!!) and thus, I copied each and every single files which were left out. (Phew!)

Once i did that, i was re-directed to "System installation" page. Just hope this will help those newbie out there like myself.

Hey, I've got an issue. I managed to set up the forms and i can receive the info in my mailbox. However, those attachments which were submitted were NOT shown in my email. I did insert the "Browse" tab on my form. Any advise?

Hello, decided NOT to have the option for customers to upload any attachments in my forms after considering the risk of pple uploading malicious docs.

However, (opps! sorry!) I have another issue again.

I wanna have 5 forms in which all of them have the same content. That is, duplicate copies of EXACT original form but just that each and every form have different path names.

Is there a way to "copy & paste" from the original form so that I dont have to re-do the same thing over & over again for 5 times?

Appreciate any help...

P.S. I am using FMP. My username: qssdkirl, ID 13354

I hate to think I'm missing out on this incredible new tool, and exactly when I needed it, too! But I seem to be the only one around that doesn't know what "chmod" means. I created the database in MySQL, and I think I have the FMP folder uploaded correctly, but I haven't "chmod"ed anything to the required 755.
This is the error I get when I use the install:
Template Error: set_root: tpl is not a directory.
Halted.

Should I start over, or am I just missing something simple?

Usually the FMp folder is by deafult set to 755, the instructions relevant to change mode are only a precaution step to make sure that everything is ok.

To do so, please connect to your site with BlueFTP and right click the FMP folder. Then select "Permissions" . If it is anything else than 755, change it to 755. That's all.

However, the problem you are having is not a permissions issue: this is a typical error that you get when NOT ALL files / folders have been uploaded. Blue FTP sometimes misses some files / folders. Check it.

Template Error: set_root: tpl is not a directory.
Halted.

Any ideas what I am doing wrong, permissions are set to 755 so thats not it.

chmod: this regards ge FMP folder permissions: conect to your site with FTP (or File Manager, through your CP and select the FMP folder. Right click and select permissions, then set permissions to 75 if it is anything different.

you can see that i explained in detail how you can fix your permissions problem.

If you want to download ABVFP, you will still have to learn to change permissions, because the installation process requires that you change several file permissions.

It is an excellent program and I use it for my forms, as do many users. It will require you to read and follow many instructions to a tee for you to use it properly. It took me several hours to read it all and get it all done correctly, but it was well worth it. I have never used FMP so i cannot comment on it.

Hi Guys,

I have read the threads so I checked that all the folders and folders within folders uploaded to my site.
The fmp folder has 755 permissions.
The database has been created.
I did initially miss off my site username in front of the data base name but that has been changed.
The My SQL host is set to localhost.


When I hit the install button I get the message Could not connect to MySQL server.

Any body got any ideas where I am going wrong?

Can Form Maker handle multi page forms dependent forms? By that I mean the ability at the end of a page to give the user the option to Submit or go to the next page.
It seems to say so but doesn't explain how to.

I'm revising a form that is used to collect data for quoting custom product, I'd like to make it possible for the user to enter 3 or 4 quote requests - every one on a separate page.

I am not sure about FMP, though i had read something about this being possible. However, look at my signature on how you can create and process a multipage form.

I also asked them direct and they came back with this:

Yes, you are able to perform such form using Form Maker Pro. There is a method called IFCOND ("if" condition) and it is used in page and email templates only.
It allows selecting the data output depending on some condition of user-entered parameters.
General syntax:
[<ifcond((condition),(output if yes),(output if no))>] where condition can be [parameter] {<, > or ==} "value"

Now all i have to do is figure that out - if you can figure it out it'd be much appreciated. I have asked them if they have any more info.

I'm aiming to put a Yes/No question at the bottom of the page like this
Do you want another Quotation? Yes/No : if Yes go to Next page, if No then Submit form

is there a problem with Form Maker Pro? I just d/led it, haven't tried it yet, am trying ABVFB first. Is one easier than the other? I just want to create a simple "lead capture page" that gets forwarded to my autoresponder. Is this possible with either software, (Form Maker or ABVFB)?

I like ABVFP better. I tried the Form Pro and think it is more complicated to do the forms, with ABVFP you do them in BV the way you want it.

Usually autoresponders provide their own code for the form to use, and you simply copy / paste. Check it.

Im still learning cpanel, although I have been able to upload files etc, I didnt know software as such for my use??

Hi all, am looking forward to making my first form. had it all install without much of a hassel. Took me two tries at the instalation page. The pathname got me raising an eyebrow, thank you General VH & Naveldesign, your help is neverending.

My one question is after the install I deleted the "install.php" as instructed, but am wondering if I leave the folder permissions at 755?

Also I noticed that that alot of people are getting confused at the instalation page, maybe the fmpdocs page could be edited to include something like below, this is what worked for me.

After you create the database for FMP, scroll down your mySQL databases page (controlpanel>mySQL databases), you will see a list of your current databases. Listed should be the database name and user for the fmp database you created. Please take note of these as they are the ones you use on the FMP instalation page + the password you made when making the database.

I followed the instructions as described in the "How to create a simple one page form" popup. I followed the steps 1-4, but i cannot figure out, where is the Get Pages button..I can only find the Get Link button.

I have an issue with one of the Form Maker Pro forms I’ve created that contains a video. All of the “Get Pages” have been uploaded to the server. On the form I made I created my “thanks” page with Blue Voda and uploaded it to the server. On it there is a video designated by the image of a slide. When I test it by putting the URL in my browser everything is fine and there is the image of the video slide.

When the form is completed and submitted, the “thanks” page doesn’t show the image of the video, instead it says “image” with a box and a red x. The video works fine though.

Does anyone have a solution for this problem?

1. I can enter information on it, but when I submit I get a 500 error;
2. I've would prefer to use the html on my own page but can't seem to get the link to work;

After finally working thru the installation and reinstallation process. I finally thought all was right and during testing I received the following 500 Internal error:
Internal Server Error

The server encountered an internal error or misconfiguration and was unable to complete your request.
Please contact the server administrator, webmaster@mybestrateloan.com and inform them of the time the error occurred, and anything you might have done that may have caused the error.
More information about this error may be available in the server error log.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

Any Ideas?

I have followed all the steps stated on http://www.vodahost.com/fmpdocs/ on how to install form maker pro, and i've got to step 5:

When I enter this with my domain in, it comes up with the good old 'The webpage cannot be found'.

Does anyone know what i've done wrong?

Hi,
I too kept getting and error 404 message when attempting to install.php.
Checked all the files had copied across and also found 2 sub folders had not.
Transferred those two missing folders.
Then went through thoroughly to check everything was copied over.
Now I find in the js (sub dir) > editor (sub dir) two more folders are missing, and no matter how many times I try to drag them accross they just wont go, and they are the file manager (sub dir) and the skins (sub dir).
Any suggestions?

I followed the following instructions. When I get to #5, I receive the following message:

Not Found

The requested URL /fmp/install.php was not found on this server.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.


Step 1: Download Form Maker Pro from VodaHost.

1. unzip Form Maker Pro and extract the fmp folder to your computers desk top. If you do not have WinZip installed on your computer click here now.
2. Login to your control panel and create a mySQL database. If you do not know how to do this, click here for full instructions.
3.Upload the entire fmp folder into your public_html directory (including the folder). You will need FTP software to do this, BlueVoda has a built in FTP manager. or you can use any other FTP software like SmartFTP or ws_ftp.

public_html/fmp
4. chmod the the entire fmp folder to 755 unless you are on the voda2 sever IP 70.86.30.66 you will chmod the the entire fmpfolder to to 777
5. Run "install.php" with your web browser. Example >>>. http://www.YourDomain.com/fmp/install.php

Hi, Just been reading through all the problems others have been having installing Form Maker.
I've got as far as trying to do the actual install (done the FTP and checked all items transferred etc).
When I try to ' ..../fmp/install.php I get an error "The requested URL /fmp/install.php was not found on this server."

Can anybody help me get through this stage??

Here I am again still trying to figure out FMP.... I paid the BV guys to install it so hopefully that will solve a major issue in me doing that part. However, where I'm stuck is finding the 'Get Link'' button - I see ''Get Pages'' are they suppose to be on the same edit page?

Am I understanding the directions correctly from the FMP Users Guide -
'' To upload your web form pages into your site:

1. Go to the form editing page and click the GetPagesButton button to generate pages;
2. You are now able to save a .zip archive which contains your form pages and readMe.txt file with addresses to upload generated pages into. If you don't like addresses provided by the system you can easily set ones of your own. To do that go to the form editing page and click the edit icon EditIcon in front of each page.

Use any ftp client to upload web form pages to your server
You can implement generated forms into your design or leave them as they are.

You do not have to upload your pages in case you use Form Maker Pro v4.2 and have created your web form on our virtual "Form Maker Pro server" site. Click the GetLinkButton button to retrieve the link to your form. Copy the link to the email or post it on your site.'' So from thisit sounds like all I have to do after creating my form is to get the url and paste it on my page - is this correct?

I am having a problem uploading the db.conf.php file to the fmp folder. I have notice that the file is listed as 0K in size. When I try to install FMP I see the error message "File db.conf.php is not writable" Any suggestions?
