Thread: form not working?

ive got 1 that works fine as a contact form and the other as a contact form exactly the same form ( ive just changed the nema of it to answer) but it doesnt work i only get 2 out of 5 possible answers i.e it gives me where they seen us and first name , it was working fine until i tested , ive just gone tthrough and double checked its the same as first then re-submitted it still nothing , oh and ive just checked and the reset button isnt workin now aaaaaaaaaaagggggghhhhhhh
