Default Form created with Form Wizard not working

Hi, my domain name is; americaneaglesigning.com. I just uploaded my web pages and they all seem to work except the last one called "Contact Form". I created this form using the "Form Wizard" and made sure it was in php format and not html. As you will see, it didn't work, Any help would be greatly appreciated.

First the success page cannot be found
Second, change the emailfield from text to email
    Third, use the verification functions.

lol I guess Im kinda Bluevoda Illiterate, Altough I think my Website is good.

By the page extension do you mean :

From - .HTML to .php, only reason I ask is because on the tutorial it says make it a .bvp

then make sure every link has that php or (bvp) extension, that are concerned with that page.

Normally every page is saved by bv as .bvp
    but
any form created by the form wizard is saved as .php
    hence the clarification.

    Hi again,
Well I got everything working just fine but then I thought I'd get creative so I downloaded the "Advanced Blue Voda Form Processor". I actually got through it to the point where it said "Congratulations". I think I followed the instructions pretty well but its not working. After creating the form, I cut and pasted it into my existing Contact Form (after of course deleting the previous one). Anyone got any ideas why its not working?
