Error 404 comes up when form submit button clicked

Hi,
My domain name is www.upthem.org

I've created 2 form pages with success and error pages however when I click on the submit buttons for both forms I receive the message "This link is broken." I submitted a request for help and was told to "make sure I had set the correct permissions to the files." I don't have a clue what this means or how to go about it. Can someone please enlighten this technological fool. Thanks

forget the file permissions thing

Your contact.php can not be found, make sure that you have published it and that the link is correct.

could not find your newsletter either

Thanks for the suggestions. The contact page seems to work okay now but I still can't get the form page "ask_alan" to send back emails although the "success" page comes up when the submit button is clicked. I'll keep working on it in the meantime.
