Youd need a database that was searchable and have the results display there. Most likely it would need to be customized. Id recommend Naval or Watdaflip for scripting up something like this. Karen

You can use a contact form where they would enter their info and it would be emailed to you to do with what ever you need. If you want something viewable on the web when they enter info you could use a guest book (you can title it to suite your needs) if you do not want to enter info on the page manually. There are videos on each of these options.
