Guestbook Problem - I cannot get it to work!!

I'll try again:
My my URL is - www.benefitplace.biz. I am not able to make the Guestbook function on the specific page - www.benefit-place.biz/contact-us.html. Per Blue Voda's previous suggestions I have tried to:
1). Review the tutorials
2). Delete and re-publish
3). Attempt to use the Blue Voda FTP and find the PHP file 'guestbookdb.php' and change the permissions - I was told in an additional Blue Voda email that the PHP was already downloaded

Questions:
1). How do I get the Guestbook to function on page: www.benefitplace.biz/contact-us.html?
2). Do I need to do anything in the FTP and download or upload - If so, where and what do I do?
3). Do I need to do anything with notepad to make guestbook work, and if so, what and where?
I am obviously missing a step in the set-up! I have been trying to set-up the Guestbook for over a month!
Please Help!

well i see the login there for the guest book, and a place to enter my comments. Clear your cache

you clear the internet temp files using explorer or use a cleaner like cc cleaner. How long have you owned a computer?

I cleared 1500 temporary files! The computer is less than a year old with plenty of memory. What do I do next?

Hi - Your message is on the site! Can I test it and fill out the form and submit? Should it show up at the bottom of the Guestbook as yours did?
