Thread: Small Form Issue

Hi all,

I having trouble placing a "Request Call Back" form on each page of my site. Ideally it has Name, Tel, Email And SEND!

I placed one on the homepage, no problems, but when i altered the content and saved as a different page i get a weird php text appear at the very top of page!

Is it because the 2 forms are the same/copied and conflicting?
I dont really want to have to create a seperate "call back" form for each page and any ideas would be appreciated.

Ok....Solved this myself! Me being a Numpty, i had published the page as .html rather than .php extension. It all seems ok now!
