Default Unwanted HTMLat the top of published page.

Been having trouble with a form page but the trouble is not with the form itself so I thought best to move my quest for help over to this area. Please have a look at the volunteer form page and tell me what is going on? Is the chunk of unwanted HTML text a virus of some sort? In awstats I've seen a strange amount of activity from Romanian sites over the last months, could this have anything to do with the problem?

    Thank you,
    Here is the page:

All "Form" pages must be published as ".php" and yours has been Saved/Published as ".html" .....

    1. Start by removing potential page conflicts by DELETING the current ".html" page from the server.
    - From FORM PROPERTIES (double-click on Form grid on your page to open):
    2. Check that for "Encoding Type" you have selected "multi-part form data"
    3. Check the box "Use built-in PHP form Processor ..." and properly complete the entries required.
    4. Re-Save your page, and re-publish.

Getting an Error 12003 when I try to complete your instructions !?
Do I save the form page normally or do I add the .php before saving?
Either way it doesn't want to publish this morning...

    Thanks, as always,

This may be redundant....you will have to change your (right click) Page Properties to .php.
If you have already done that open your Blue FTP and manually delete the html version.
