There are many commercial scripts that can do what you want at a moderate cost. Search for "Business Listing" or "Business directory" scripts.
Creating one on your own is out of the question without extensive php/MySql knowledge.

I have built many, using BlueVoda for the visual part. If at the end you don't find one that suits you, you can contact me. The cost for a custom made script, can be anything from $300 (in the simplest cases) to $500 in more robust ones.

Seems like they fixed the DNS issue, but now I am having email issues. For some bizarre reason I can receive email but not send it. Are you having this issue too?

There are mistakes in your forms. The encoding type is text plain instead of the correct multipart/form data. Many fields have the same name (like Activities). Last, there is absolutely no mechanism to pass values from one page to the other, so you only recieve the values of the last form (page 3).
To split the survey in 3 pages, you need a multipage form. HAve a look at Multipage Form Tutorial

You should look for instructions at the same place where you downloaded it. Debugging an entire script that someone else wrote, is a very difficult operation and i am afraid i don't have the time to do so.
I have a similar script of my own, but very user friendly and with full instructions, support and demo. Have a look at http://www.dbtechnosystems.com/logger_lite.php

Their site doesn't have any information on it. I can tell from the screenshot that this script will require you to change your pages you a .php and add some php code to each file to protect the page. I would assume there is a readme file that came with the download, the php code you will need is most likely there.
