    Default Adding a form

Hey, When I add a form to generate an Email, the php script for the form displays on the background of the page and when the submit button is pressed it attempts to redirect to this url http://www.jbntelco.com/%3C?%20echo($_SERVER['PHP_SELF']);?> instead of www.jbntelco.com/success.html or www.jbntelco.com/error.html... anyhelp is appreciated.. thanks
