Default Setting up form maker problems

have set up the fmp file and uploaded the contents of the file. Have changed the access priority to 755 and have trid to activate using http://74.52.103.210/~mbyfwvbj/fmp/install.php however I get the following error message
    Internal Server Error

The server encountered an internal error or misconfiguration and was unable to complete your request. Please contact the server administrator, root@voda10.totalcontrol.host and inform them of the time the error occurred, and anything you might have done that may have caused the error.
More information about this error may be available in the server error log.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

    Hi Robert,
This address looks like the temporary address from your welcome letter.
    http://74.52.103.210/~mbyfwvbj/fmp/install.php

Have you published your index page yet?
And have you created a database for the fmp?

    Thanks for your reply

This is the temporary address as I am waiting for the DNS settings to be revised with my original name server as my domain name is presently re-directing to another host.

Yes I have published and index, which i can vew if i goto the temporary address and yes i have set up a databse using the control panel.

Its posible that all the fmp files have not been copied to your fmp directory, in that case you may have to check each dir and each file within the sub directories.
Have you tried to put in a support ticket ?
