Thread: FormMaker Pro help

Could someone please tell me how to correct my preview button on my contact us page: http://willdigforu.com/contactus.php
I have submitted a ticket on VodaHost help, but they told me it was a technical issue they could not help me with.
After clicking on the radio button titled Preview, I am given an error message that says URL not found on server.
I have tried to go to the website where I created the form, FormMaker Pro, but I can not access it for some reason.
My contact us page is useless unless I can correct this problem.

Have you downloaded the contact page & then uploaded to your server/site so you can preview.??

SYSTEM INSTALLATION

Could not connect to MySQL server (Here is where things go south!!!)

It is asking for:

a) Enter your username, email address and password in corresponding fields;

b) Reenter your password to confirm it in the "Confirm password" field;

c) Enter your MySQL database full path in the "MySQL host" field;

e) Enter your MySQL database login and password in corresponding fields;

f) Enter your MySQL database name in the "MySQL database name" field;

what goes in these fields????

Sorry, My other computer is down and getting repaired. That's were I keep track of goings on...
