| Hi, I installed form maker pro a few days ago and found the thread in the FORMS FORUM here on this site very good. On there Allien posted the following If anyone is stuck on the first page it goes. localhost username which is your cpanel username password which is your cpanel password database name which will be cpanel username_databasename which you created. It worked great for me, dont know whats causing your database error though. Best of luck, Mike

| This doesnt belong in the Soho thread, and it wont work with Soho launch as soho has its own form builder installed. Karen

| Hi, not sure if I am reading this right? But have you created the user name & password in the MSQL area for the database. This is separate to your normal login details.
