Red face Form Maker Pro Help

This is a message for Navaldesign or whom ever can help. I have read a lot of posting and still in the dark a little.

I understand this much so far.
1. I have done Downloading the software.
2. went to Cpanel and form a data base
3. uploaded software with ftp server.
4. now I am at the system installation is where I am having the problem.

username;
e-mail
password
confirm password
mySQL Host
mySQL login
mySQL Password
mySQL database name

I have read where navaldesign posting where it was advised to.
"Host will be "localhost", mySQL Username and Password will be the ones that you will set during the creation of the database."

It is so many passwords and user name floating around in my head, Boy""
I went back to cpanel and retrieved where I created my database.
Examples: DBI:mysql: gouwe_rock
dbh=mysql_connect (localhost, gouwe_stone,<Password>
and i see a funny error massge on my, "MySQL Account Maintenance"
" "<PASSWORD HERE>") or die ('I cannot connect to the database because: ' . mysql_error());"

I know in my welcome letter I received a lot of info. is this where I get the username and password.

Hi,
I installed form maker pro a few days ago and found the thread in the FORMS FORUM here on this site very good. On there Allien posted the following
If anyone is stuck on the first page it goes.

localhost
username which is your cpanel username
password which is your cpanel password
database name which will be cpanel username_databasename which you created.

It worked great for me, dont know whats causing your database error though.

Thanks for the response, You have cleared a few items but I am still having a no/go at the station. I have found the cpan username & password. the database is good.
on the e-mail, is it the default e-mail from my website,, mail@cvgdesignphoto.com or the e-mail i sign up with cvggraph@mac.com
where does the MySQL host come from, is it my name underline the 3rd line in cpan, user in gouwe or gouwe_stone

This doesnt belong in the Soho thread, and it wont work with Soho launch as soho has its own form builder installed.

Hi, not sure if I am reading this right? But have you created the user name & password in the MSQL area for the database. This is separate to your normal login details.
