Thread: Order Form not complete for customer

I had gone into my website to 'place an order' as a customer would and found that the complete order form that the patient would fill out is not showing.......only the top part. The order form is complete on the back end, so there's nothing missing....it just doesn't show all the fields for the customer. Upon checking a few weeks ago, there was not a problem, but there is now. Has a problem occur since then or is there something else going on? Please advise on what I need to do to get the complete top to bottom form to show for the customer. thanks.

Thanks for your quick response to my delema, however, I figured out the problem. I had two forms created; i apparently had updated the form and saved it as a copy instead of just 'saving' the updates. All of the products on that page were linked to the original form that did not show the new fields.
