I published a form to my site www.projectmercy.com/contribution.php, however when the info is sent to my email it is not collecting ANY of the radial button information. What should I do to correct this?

Have you defined the properties for the buttons...not the label?

double click on the radial button in the form ==> a properties box will appear and should have two values...one the NAME & the over the VALUE you want it to return ie. $25.00
