FormMaker Pro installation not working

Hello, I have been trying to install Form Maker Pro. Starting from the Treasure Chest forum I followed all the links and instructions step by step.

The following is the Installation Instructions copied directly from the instructions page:
Step 1: Download Form Maker Pro from VodaHost. Done that!!
1. unzip Form Maker Pro and extract the fmp folder to your computers desk top. If you do not have WinZip installed on your computer click here now.
I have completed this step!!

2. Login to your control panel and create a mySQL database. If you do not know how to do this, click here for full instructions.

I have completed this step!!

3.Upload the entire fmp folder into your public_html directory (including the folder). You will need FTP software to do this, BlueVoda has a built in FTP manager. or you can use any other FTP software like SmartFTP or ws_ftp.public_html/fmp

I have completed this step!!

4. chmod the the entire fmp folder to 755 unless you are on the voda2 sever IP 70.86.30.66 you will chmod the the entire fmp folder to to 777
I have completed this step!! Although all the sub files and folders in the fmp folder have not changed to 755.

5. Run "install.php" with your web browser. Example >>>. http://www.YourDomain.com/fmp/install.php

This is where I get into trouble and get the message:

Template Error: set_root: tpl is not a directory.
Halted.

6. Fill in the "System Installation" page fields:

Any suggestions would be appreciated.

Some files / folders are missing. So NOT use BlueFTP (it has the bad habbit to forget subfolders)

Check it yourself to make sure.

So are you saying to do it through cPanel rather than Blue Voda's FTP manager?

And what files and folders are missing? How do I know what's missing and where to find them? Did I do something wrong during the process?

I know you're very busy but I need a bit more of a step by step instruction. Perhaps just when you have some time you can get back to me.

File Manager is even slower and you must upload one file at a time.

Use this one.

Oh, ok. I uploaded the fmp folder containing all the files, didn't realise it had to be done one at a time. They should be more specific about that, there's a lot of files in that folder.

I had similar problems. The tpl error was due to FTP not uploading the tpl folder along with the others - shouldn't BlueVoda fix this bug??. That's now fixed and I am trying to get the install working.
I have read through lots of forum responses but I am still a bit vague on some of the System Installation fields, since I keep getting "Could not connect to MYSQL server" -
'My SQL host' - is this the IP address of my server? (ie. '208.43.222.136')
'My SQL Login' - is this masterusername_SQLusername or should it be masterusername-SQLusername, or something else?
Similarly for My SQL database name.

But, unless you need some of the specific features of FM Pro, you can use the built in form processor, or ABVFP. Much easier, better control of the look as the forms are created within BV.
