Having problems installing Form maker

I followed exactly the instructions given on http://www.vodahost.com/fmpdocs/. After uploading the fmp folder to my site and change to permission to 755, these message are shown. please see, http://www.dial-international.com/fmp/install.php

This is a typical error. Not all files / folders have been uploaded. Check it. Then delete everything and start from scratch.
