error message in form

I have watched the tutorials..have tried and tried everything I can think of, I still get this message when testing my email setup to capture emails and names in a form.
Parse error: syntax error, unexpected T_STRING in /home/dajoxcyv/public_html/process123.php on line 11

what am I doing wrong?

You have probably missed a ";" in line 10
