Disappearing file

On a form, I have a file upload button for people to send me a picture.

I've tested it and all the fields operate, but when a pic is uploaded, it seems to go, but not to the same place as the rest of the information.

In fact it seems to have gone off into the atmosphere somewhere, 'cause when I search the computer for it it isn't there.

Any ideas anyone?
