Please help...submit button not working

Hello,
I have created a form, following the tutorial very closely, with 2 radio buttons and a submit button. When I preview it and hit submit I get the following message.

Firefox can't find the file at /C:/DOCUME~1/HP_OWN~1.NEW/LOCALS~1/Temp/preview/<? echo($_SERVER['PHP_SELF']);?>.

I have tried shutting my computer down and even using a different browser but I get the same message.
The page name is http://silverbrasforcancer.com/purchasebra.html

I looked in my file manager and there is one there with .html and one with .php...sorry I am a real beginner.

Since you have a form on it .. it must be changed to the php extension. Thats why its not working and thats why that stuff is coming up on top. You need to use the php one.
