Need help with creating custom email form

Hi,

We are in need of someone to help us to create a questionnaire/contact page with the data emailed to us. Please advise your charge for this and any further questions you may have. Our Voda Host domain name is ksdmarketingconsultants.com I have a questionnaire up on http://ksdmarketingconsultants.com/contact1.php but of course I cant figure out the problem why it isn't emailing data.

I am sure many people here can help you, but you can help yourself and use this insanely easy forum building/data collecting website called wufoo, there's a free plan or a paid plan. They have an integrated form builder and keep all the data for you (you can make charts and DL the info to Excel).

You can also use VodaHosts Form Builder Tool http://www.vodahost.com/forms.htm if you want the data with your service provider.

Thanks for that info Robert.. I decided to go with the BV Pro Form Builder and have them upload it for me, need the quickest way now.
