Erratic performance of form

I've published a contact page (BV11) which most of times works fine. However, I've been receiving emails with two different types of errors: in one of them, the text is all scrambled as shown below (editboxes 3 and 4 should show numbers):

In the second type of error, the text is ok but content of editbox 2 (email address) is replaced by content of editbox 3 (telephone number) and editbox 3 shows content of editbox 4 (date of birth) - which means there is one editbox missing...

When I send the contact page from my own computer, email received is correct. Can anyone help me fixing it?

usually thats a bot that has filled them out. Thats why the erratic nonsense.

Thank you, Karen. I understand that could be the reason for error #1. Do you have any idea about what could be causing error #2?

maybe you copied the box and it kept the old names or positions?
