More question about form, Help Please!

Maybe Ill get this one answered. I have two drop down box on on my form wizard. I am getting the email just find, but the form is only reading one of my drop down box instead of both when I put info in both. Yes they are both on top of the form. How do I get the form wizard to read both. I need at least a total of 5 drop down box's. I am using html for the drop down box.
