Form not working...

Hi...

I have made a form, which is supposed to allow the user to upload a picture and i will receive an email with their details (from the form) and a link to their picture. I have had this working and now can't understand why it isn't. It just goes to the error page.
Also it looks as though their is some html coding stuff at the top of this page...sorry, quite new to this! any help appreciated!

You have two identical pages, one with html extension and one with .php extension. The correct page is the one with the .php extension: http://www.thphotoshop.com/orderframe.php

Delete the .html one from the server, and edit your links to point to the .php one.
