Form wizard form not sending or resetting

Could really use some help.

I created a contact form with form wizard a few months ago. It works fine.

I made another with form wizard recently and can't figure out why it won't send or reset.

Because ALL three fields as well as the reset and the submit buttons are outside the form boundaries.

Stretch the form to cover all elements. Then move each field just slightly, untill you see the form border highlighted, which indicates that the field or button have become part of the form.
