3 pages,1 form how do i forward it all to outlook express?

hi
i've watched the tutorials, and read all the threads to set up an e-mail form, with reply page. i've set up email accounts in cpanel & outlook express and now have a page with the form on, a php page to put in that page(but don't know how-the tutorial says put this page in the same file as the page with the form on but i can't figure out how) and a reply page thanking people for their enquiry. Please can someone tell me how to tie them all together so i can get this site online and working???
