Forms message 404: can't find .php when form submitted; parked domain the cause?

I've watched the forms videos and tutorials many, many times and searched the forums as well. When my form is submitted (www.planningtoolkit.com/planningtoolkitcontact.html) a 404 message comes up, stating that the form (planningtoolkitcontact.php) cannot be found. This is a parked domain. Could that be the problem or am I missing something?
