As you have a FORM on your "index" page, you must re-name the FORM itself in FORM PROPERTIES from "contact" to "whatever-you-decide" for it to be recognized and able to process the script, as described in Step 7: I would suggest you re-name it as "susbcribe" (since that helps you identify what FORM it is supposed to be), and that you reflect this in continuity between all your pages (Success, Error, etc.) and re-publish them all accordingly.
