Your page has been published as ".html" rather than ".php" as required when any FORM is on the page.

1) I could understand because my form is different from the link you give me. Please give me the real answer.

2) Why there is no change after I save the page & publish again the page. I hope the page is like as I save the change but still before after save. Why?
