problems with form wizard

I am creating my "contact us" with form wizard and using the business contact template from the list
once completed and published i had code words appear at the top of the screen and are still there
So i started a process of elimination and found that the problem was the form wizard itself
Btw i tried other form templates and had the same problem occur

Please make sure that in the page properties you have set the page extension to php. Right click on a blank area of the page, select page properties, then use the drop down arrow to select php. Save, then publish again. You will need to delete the contact.html page as well. If you need assitance with that you should open a support ticket.
